If you are invited to interview, please print and complete our Criminal Convictions Form, using the the accompanying Recruiting with Convictions Policy to help you. Forms should be submitted in a sealed envelope with your name and specifying a Criminal Convictions Form is enclosed. This will only be opened by our HR manager if you are offered the post, otherwise the unopened envelope will be securely destroyed. For form and policy please see the boxed links.
If you are offered a post at FDAMH you will need to join the Protection of Vulnerable Groups (PVG) scheme. This is a criminal records check and you will be invited to come to FDAMH to fill in and provide the necessary documentation to progress this.
